Hospital Discharge Checklist

When an older adult is leaving the hospital, families often have very little time to organize medications, follow-up care, equipment, transportation, and safety needs. This checklist helps families slow the process down and ask the right questions before discharge.

Before Leaving

  • Ask for the written discharge summary
  • Review the diagnosis and reason for hospitalization
  • Confirm who to call with questions after discharge
  • Confirm transportation and where the patient is going

Medication Safety

  • Review every medication change
  • Remove discontinued medications at home
  • Confirm prescriptions are filled before arrival home
  • Set up pill box or medication reminders

Follow-Up Care

  • Schedule primary care follow-up
  • Schedule specialist visits
  • Confirm home health or therapy orders
  • Know warning signs that require urgent help

Home Safety

  • Clear walkways and remove throw rugs
  • Check bathroom safety and lighting
  • Confirm safe access to bedroom and toilet
  • Decide whether home care or supervision is needed
