When an older adult is leaving the hospital, families often have very little time to organize medications, follow-up care, equipment, transportation, and safety needs. This checklist helps families slow the process down and ask the right questions before discharge.

Before Leaving

Ask for the written discharge summary
Review the diagnosis and reason for hospitalization
Confirm who to call with questions after discharge
Confirm transportation and where the patient is going

Medication Safety

Review every medication change
Remove discontinued medications at home
Confirm prescriptions are filled before arrival home
